Choose the decision boundary

List the actions the system may take independently and those that require approval. Be specific: creating a draft record may be acceptable while submitting the record requires review.

Show what needs attention

A review task should include the source, the proposed action, the uncertain fields, and the reason it was escalated. Simply placing an item in a queue does not make a decision easier.

Plan the exception path

Decide who receives failures and what happens when that person is unavailable. Include timeouts, retries, duplicate protection, and a route back to the original information.

Keep authority visible

Permissions should match the task. Avoid giving an assistant broad access because it is convenient during development. Separate the ability to read, draft, approve, and execute where the workflow calls for it.

Make improvement a routine

Review exception patterns with process owners. Some point to poor input quality; others reveal a missing business rule. Changes should be evaluated against representative examples before release.
